This page is a quick reference for Alpine Christian School families. Complete enrollment and health information is also available in the ACS Parent & Student Handbook 2026–2027.
ACS reviews its student health and enrollment requirements in accordance with current Illinois State Board of Education (ISBE) requirements. Families will be informed when changes affect requirements for their student.
This webpage is designed as an easy-to-use family checklist. The ACS Handbook and applicable Illinois requirements remain the primary references for complete policies and requirements.
Every new student
starts here.
New and transfer students should begin with these enrollment essentials. Additional health requirements depend on grade and previous school enrollment.
Birth Certificate
Provide an official or certified birth certificate, or other legally acceptable documentation when applicable.
Immunization Record
Provide a current record showing your child’s immunization history in accordance with applicable Illinois requirements.
Previous School Records
Students transferring to ACS will need appropriate academic and school records from their previous school.
Consent to Treatment
A completed Consent to Treatment Form should be on file so ACS has the appropriate information for student care.
Two References
New and transfer student applications include two references as part of the ACS admissions process.

Enrollment forms are completed online through AE Connect. This checklist helps you prepare the supporting documents that may also be required.

Required dental examination documentation is due for students in Kindergarten, Grade 2, and Grade 6.
